Medical Management, including Health Coach and Medical Advocate
Medical Management, including Health Coach and Medical Advocate
“Physician in the Family” was created to provide personal support, advice and counseling to you and your family during life’s passages. We are your partner in facing immediate crisis, or in a chronic condition requiring long-term care, or in preventing and minimizing the chances of any diseases or injuries, or in wanting to achieve peak performance and health.
The face of medicine and healthcare has changed dramatically in the last fifty years. We have progressed from only a handful of drugs for any given condition to a multitude of new medications. Crude surgery has advanced from open exploratory procedures to less-invasive arthroscopic surgery and more-precise robotic surgery.
And with these changes, so has the general family physician who did everything has become a relic of the past. One of my most memorable experience was an externship in the Sacramento Delta with the town’s family doctor. A warm and devoted man, years later I returned to find that the town named the city park after him. after his death.
Yes, the doctor who delivered babies, and performed orthopedic surgery and rounded on his patients in the hospital is not to be found today in the large metropolitan areas. Private practice has been replaced by large medical groups with time constraints on the patient visit, mid-level providers of nurse practitioners and physician assistants used for cost-effectiveness, an enormous amount of legal paper work, and hospitalist physicians caring for you in the hospital. Though supposedly not legally allowed, we are indeed in an era of Corporate Medicine. Though a few physicians have chosen a concierge practice, the future wave of large medical groups intertwine with hospital corporations, is now well established.
While a systematic healthcare organization directed by insurance companies and hospital corporations have many advantages, it comes with a more complex and confusing system, and less personal physician-patient relationship. As a general model, when one speaks of Kaiser, there is an insurance company, a hospital corporation and a medical group. Needless to say who has the least say. Indeed, when a patient is asked who there physician is, the response is Kaiser and not a physician name.
Physician in the Family was created to fill the void of the weakening physician and patient relationship. We are not a concierge physician or primary health doctor. Instead, we are your medical counsel, serving as your health coach laying out the strategy with your physicians, your partner in educating you on aspects and options of your healthcare situation, and your medical advocate to assure the best medical management by helping navigate the insurance and hospital care maze. We do this with a team approach that includes interacting with your healthcare providers, your present insurance and hospital system, along with our team of consultants, and with you at the center as an active participant.

Mary had been independent and healthy all her life. She lived alone, but had strong church support and distant family support. At age 83, she was overwhelmed by the diagnosis of esophageal cancer and the treatment options. She wanted just to give up and not get any treatment, and her family went along with her hopelessness. Family in the Physician served as Mary's health coach to educate her on the treatment options and make recommendations, and served as her medical advocate in coordinating with her medical providers and facilities to insure that she got appropriate and timely care.
Victor excelled in the athletic arena. And when last season's injury and recovery had become a concern to his future contract, he understood enough not to go to the team physicians and turned to Physician in the Family. He appreciated the discreet and honest assessment, education, and personalized treatment recommendations. He felt comfortable in knowing that care was being coordinated with his needs and concerns foremost, in with the utmost confidentiality.
James had been a CEO of his own company, and had always been in control of his life. He was a world=traveler, frequently travelling between the US and Europe for business and pleasure. ,A man of action, he remained active in organized sports even to his mid-80's. Unfortunately, past lifestyles and heart conditions caught up and further deterioration of his heart impacted not only recreational activities but also daily activities. Despite having the resources and access to the best medical care, he was not satisfied with his unanswered questions nor the slow pace of medical attention. James turned to Physician in the Family to answer his medical questions. We then became his healthcare advocate to make sure that he found the most qualified specialist to handle his procedures. We were there for any complications or turn of events, to assure that he got the access of care and attention that he deserved.
